Request a vaccination certificate excluding COVID-19

DHA’s vaccination-certificate service issues a certificate for vaccination records held in HASANA, excluding COVID-19 vaccinations. On the DHA website choose the general vaccination certificate form and provide one of the requested identifiers: medical record number, Emirates ID or passport number. A vaccination record must exist in HASANA for this service. The service is free. COVID-19 certification uses a separate form. This is a record request, not advice about which vaccines a person needs or proof that an unrecorded vaccination has been accepted.

DHA death notification, certificates and replacement copies

DHA’s death-certificate service includes new certificates, replacement copies and certificates for deaths outside a hospital when the deceased is brought to a hospital. The checklist includes identity documents for the deceased and recipient, a police no-objection document, and a forensic report for a death outside a healthcare facility. Collection is restricted to an eligible relative up to the fourth degree, legal representative or person authorised by a police letter. Replacement or extra-copy requests need the recipient’s identity document and an existing certificate copy; if the certificate is lost and no copy is available, the service requires a police letter. Use DHA’s Birth and Death Notification System. New Arabic and English certificates are listed as free; extra or replacement certificates are listed at AED 70 per certificate. Confirm the documents for the specific request with DHA.

DHA birth certificates and replacement copies

For a new birth certificate, the birth hospital creates the notification, checks the original documents and uploads the application for Arabic and English certificates. DHA lists the parents’ identity documents and passports, the stamped birth notification and marriage or authenticated parentage documentation applicable to the case. Do not use this checklist to decide a family’s legal eligibility; confirm the current documentation with DHA. Parents can request extra or lost copies through DHA’s Birth and Death Notification System using identity documents and an existing certificate copy. A police letter is required for a lost certificate when no copy is available. The service lists AED 70 per certificate.

Request a DHA COVID-19 vaccination certificate

DHA provides a separate COVID-19 vaccination-certificate service through its website and app. Provide a medical record number, Emirates ID or passport number as requested. The vaccination record must exist in HASANA. The service is free. Use the COVID-19 certificate form rather than the general vaccination form. The certificate service does not decide vaccine suitability, travel-entry acceptance or whether an unrecorded vaccination meets another organisation’s requirements.

DHA medical-report attestation and its exclusions

DHA’s medical-report attestation service is for reports issued by a DHA-licensed healthcare provider. Submit a scanned official report and Emirates ID if required through the official DHA service channel. The published exclusions include reports older than six months, unsigned or unstamped reports, missing patient identity or file number, unapproved templates and reports from unofficial or non-healthcare issuers. It also excludes medical-fitness reports, invoices and sick-leave certificates. The service lists one working day and no fee, without guaranteeing completion for every request. The public card mentions website and email channels but does not name a submission email; confirm the current receiving channel with DHA.
